- OBTAIN AN A CAT ASSESSMENT
To access Commonwealth funded aged care services, a valid ACAT (Aged Care Assessment Team) approval must be obtained. See your doctor or contact us at Finley Regional Care or refer to the
My Aged Care website for more information.
- DECIDE THE TYPE OF CARE YOU REQUIRE
With the assistance of the ACAT, a decision needs to be made around what type of support services needed. This could be:
- Community based care (in your home)
- Respite care (short term)
- Permanent residential care

- CONSIDER THE FINANCIAL IMPLICATIONS

- HAVE A CURRENT WILL IN PLACE

- ADVANCE CARE PLANNING
Advance Care Planning helps you to make plans for your future medical treatment and care needs.
- HAVE A POWER OF ATTORNEY IN PLACE
If you need help managing your affairs, you may choose to give someone you know and trust the power to make decisions on your behalf. These powers will allow the person to manage your affairs when you prefer not to do so personally, or are no longer able to.
- UNDERSTAND YOUR RIGHTS AND RESPONSIBILITIES AS PER THE RESIDENTIAL AGREEMENT
Before you move into an aged care facility ensure you understand and sign the Resident Agreement and know your rights and responsibilities
