From Home Care Packages to Support at Home
The previous 4 Home Care Package levels have transitioned to 8 Support at Home classifications, offering more tailored care and flexibility.
The 8 Support at Home Classifications & Indicative Budgets
- Minimal support for independent living (light household tasks, short check-ins) $10,697
- Light personal care & social support (regular visits, medication reminders, shopping) $15,981
- Moderate assistance with daily living (hygiene, mobility, tailored meals, transport) $21,919
- Low-to-moderate support (care coordination, chronic condition support) $29,545
- Moderate support needs (higher personal care, nursing, allied health) $39,535
- Moderate-to-high support (daily support, complex care planning, medication management) $47,957
- High support needs (intensive personal care, frequent nursing) $58,122
- Very high support needs (multiple daily visits, complex clinical care, equipment needs) $77,709
(Budgets are reviewed quarterly; unused funds can roll over up to $1,000 or 10% per quarter.)
Additional Pathways
- Restorative Care Pathway -short-term allied health to regain independence.
- End-of-Life Pathway – comfort-focused care for the last 3 months of life.
- Assistive Technology & Home Modifications (AT-HM) – separate funding for equipment and home upgrades.
